Welcome to The Chicago Dental Studio Subscriptions site. By subscribing to any of our membership or subscription plans, you agree to the following Terms and Conditions (“Terms”). Please read these carefully before enrolling.

 

1. General Terms

These Terms apply to all subscription and membership plans offered through The Chicago Dental Studio via our Subbly platform. By subscribing, you acknowledge that you have read, understood, and agree to be bound by these Terms and any additional policies referenced herein.

 

2. Subscription Plans

Our subscription offerings are designed to provide ongoing dental care and exclusive benefits for members. Each plan includes specific services, intervals, and pricing as listed on our subscription site.

 

All subscriptions automatically renew at the end of each billing cycle unless canceled in accordance with the terms below.

 

3. Stay Aligned Subscription Policy

The Stay Aligned Subscription requires a minimum two-year (24-month) commitment from the date of enrollment.

 

  • Early cancellation before completing the two-year minimum term may result in a $150 cancellation fee.

  • The cancellation fee covers administrative and service costs associated with early termination.

  • After the initial two-year term, your subscription will continue on a year-to-year basis and may be canceled at any time with no penalty by providing written notice at least 14 days before your next billing date.

 

4. Payments & Billing

All subscription payments are processed securely through Subbly. By enrolling, you authorize The Chicago Dental Studio to automatically charge your chosen payment method for recurring payments based on your selected plan.

  • Payments are due on the billing date specified at checkout.

  • Late or failed payments may result in suspension or termination of benefits.

  • Prices and offerings are subject to change with reasonable notice to members.

 

5. Cancellations

You may cancel your subscription by logging into your account or contacting our support team at subscriptions@thechicagodentalstudio.com.

  • Cancellations made after the billing date will take effect at the end of the current billing cycle.

  • If your plan includes a minimum term (e.g., Stay Aligned), early cancellation will incur the applicable cancellation fee.

 

6. Refunds

Subscription fees are non-refundable once a billing cycle has begun. If you believe a charge was made in error, please contact us within 7 days of billing for review.

 

7. Modifications to Service

The Chicago Dental Studio reserves the right to modify, suspend, or discontinue any subscription plan or benefit at any time, with reasonable notice to affected members.

 

8. Limitation of Liability

While we strive to deliver exceptional care and service, The Chicago Dental Studio shall not be liable for indirect, incidental, or consequential damages resulting from participation in any subscription or membership plan.
